Responsibilities

Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
Complete face to face encounters and assesses the following: disease related changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.
Order and review appropriate lab, x-rays, EKG's and other diagnostic procedures for palliation and treatment of the terminal disease as dictated by the patient's condition and documents the treatment plan accordingly and reviews results with collaborating physician.
Make referrals to other disciplines as indicated by the patient's needs.
Actively participate in interdisciplinary team conferences.

Qualifications

Current and unencumbered license as a Nurse Practitioner in the state the employee is assigned to work by the Company.
Minimum of one (1) year experience as a Nurse Practitioner or minimum of one (1) year experience as a hospice Registered Nurse in last three (3) years.
Must have the authority to practice as a nurse practitioner in the state where services are provided.
Must have the authority to prescribe as a nurse practitioner in the state where services are provided.
Must have obtained Medicare billing privileges as a nurse practitioner.
Must meet physician-nurse practitioner restrictive collaborative requirements specific to state assigned to work by the Company and supply a copy of all active collaborating physician agreements and indicate whether these will be transferable to your employment with Amedisys.
Must have insurance required by state law for nurse practitioners to practice [and prescribe] in the state where services are provided or be eligible for such insurance requirements to be satisfied by group coverage with an employer.
Knowledge of Medicare reimbursement and coding for all levels of service - home, office, nursing home, assisted living, etc., Medicaid and other regulatory requirements.
Current CPR certification.

Job Title/Position: Nurse Practitioner Reports To: Clinical Director JOB DESCRIPTION SUMMARY The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Job Summary: Primary function is completion of face to face visits, under Physician oversight. Responsibilities include promoting and maintaining the standards of practice consistent with quality hospice care. Job Classification: Management Lines of Authority: Reports to Administrator Job Qualifications: Education: Graduate of an accredited college/university with a degree as Nurse Practitioner and is fully licensed to practice medicine. Skills: Ability to establish and maintain effective working relationships and communication with all segments of the staff, the lay, and professional public, the Board of Directors, Interdisciplinary Team and physicians. Has expertise in the medical care of terminally ill individuals. Physical and Mental Effort: Prolonged sitting is required. Requires ability to handle stressful situations in a calm and courteous manner at all times. Requires working under some stressful conditions to meet deadlines and agency needs.
