Essential Functions: Evaluation

Assumes responsibility for the face to face visit to be completed.

Upon request, attends interdisciplinary team meetings, participates in the patient care planning process and collaborates with interdisciplinary team to promote coordination of patient care.

Provides reports and assessments to the Physician, assuring that the patient receives appropriate measures to control symptoms.

Collaborates with the physician to identify needs, to ensure pain and symptom control.

Understands the CMS guidelines for Face to face visits.

Maintains current knowledge of the latest research and trends in hospice care and pain/symptom management.

Works with the IDT in a team approach, participates in performance improvement programs.

Demonstrates knowledge in communications and counseling patient/family in dealing with end-of-life issues.

Provides complete documentation to the agency timely

Provides consultation and education to colleagues and attending physicians related to admission criteria for hospice and palliative care.

Acts as a liaison to physicians and staff

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. About You To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education/Experience: Master of Science in Nursing (MSN) or higher from an accredited program. Completion of an accredited Nurse Practitioner program. Minimum of two years of Nurse Practitioner experience preferred. Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred. Experience with Part B billing preferred. Licenses, Certifications and/or Registration: Current RN license and Nurse Practitioner license/certification in state of practice. National NP certification (ANCC or AANP). Collaborative Practice Agreement with supervising internal physician or Proof of Independent Practice Authority in accordance with state regulations. Controlled Substance Agreement, if applicable. DEA registration and prescriptive authority in accordance with state regulations. Current CPR certification. ACHPN certification preferred. Valid driver’s license and current automobile insurance. Specialized Knowledge and Skills: Strong clinical assessment and critical thinking skills across palliative and hospice populations. In-depth understanding of palliative care principles, hospice philosophy, home health eligibility, and end-of-life care. Working knowledge of LCD criteria, Medicare/Medicaid regulations, coding/billing, and documentation requirements. Excellent interpersonal, written, and verbal communication skills. Knowledgeable of Hospice & Palliative Quality Measures collection, documentation, reporting, and the QAPI process. Ability to work both autonomously and collaboratively in a fast-paced, evolving environment. Compassionate, emotionally intelligent, and sensitive to the needs of patients and families. Proficient in point-of-care technology and electronic medical records. Willingness and ability to maintain productivity and documentation standards in alignment with organizational expectations. Personal Traits, Qualities and Aptitudes: Sensitivity to the impact of life and death issues faced by individuals with terminal illness; tact in dealing with these patients and their families. Ability to support others at a time of crisis. Must demonstrate flexibility by performing various tasks, often changing from one to another of a different nature without loss of efficiency or composure. Communicate effectively with people of all socioeconomic and cultural backgrounds. Demonstrate maturity, cooperation, flexibility, and tact when dealing with co-workers and all members of the hospice team.
