Pay range for this position is $36.59 - $53.17/hour. Actual pay is based on years of licensure. This is an Hourly position.


 

OSF will invest in you day 1!  External candidates with 5+ years of direct experience will receive a bonus of 24 hours of Vacation/Holiday time frontloaded on day 1.  This is in addition to our generous Paid Time Off plans!

Overview

POSITION SUMMARY: The Registered Nurse Home Care is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, therapy, social work, home care aide, home medical equipment and home infusion services as needed. The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care. Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode management.


Qualifications

REQUIRED QUALIFICATIONS:

Licenses/Certifications: Current licensure by the State Board of Nursing in appropriate state (Illinois or Michigan)

Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.

Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.

Maintains a valid driver's license.

BSN

Experience as an RN in a healthcare environment

Electronic Medical Record and Microsoft application basic proficiency

Position Summary: As a member of the interdisciplinary team, the Hospice Nurse works under the general direction of the Director of Hospice and the attending physician in the provision of skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician’s orders and interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible to provide direct supervision of Home Health Aides and or LPN’s providing care for Guthrie Hospice. Education, License & Cert: Registered Professional Nurse currently licensed in Pennsylvania. Knowledge of nursing theory and practice at a level of normally associated with an Associate's Degree. Education and experience requirements may vary according to combined background of the individual. Experience: One (1) year nursing experience in an acute care setting and/or previous Hospice experience or similar work with patients/families experiencing terminal illness is preferred. Experience and skill in supervision desired. Essential Functions: 1. Congruent with Guthrie Hospice Nursing Model, assume primary responsibility of at least a one to fifteen patient /family caseload that includes the accessing, planning, implementing, and evaluating phases of the nursing process. 2. Initiate communication of attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family. 3. Maintain regular communication with Hospice director, Medical Director and attending physician concerning caseload. 4. Obtain data on physical, psychological, social and spiritual factors that may influence patient/family status and incorporate that data into the plan of care. 5. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. 6. Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support. 7. Accept responsibility for coordinating physical care of the patient by teaching of the patient by teaching primary caregivers, volunteers, employed caregivers (LPN’s, hospice aides), or by providing direct care as appropriate. 8. Meet regularly with hospice nursing staff, and attend interdisciplinary team meetings to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth, and facilitate issues from caseload for full staff discussion, consultation and evaluation. 9. Supervise and document hospice aide plan of care every 2 weeks and supervise and document the practice of the LPN in the provision of patient care as delegated. Other Duties: 1.
