Paradigm is looking for a full-time, benefited Field Nurse Case Manager to cover the greater Detroit, MI, area.

A Field Case Manager role is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.

Qualifications

  • Current, unencumbered Registered Nurse (RN) license in Michigan
  • Experience in case management of workers comp cases, strongly preferred
  • Recommended certifications (CCM, CRRN, CDMS, CRC) strongly preferred; will need to obtain within two years of hire date
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
  • Ability to operate autonomously with minimal oversight
  • Skilled at patient education
  • Bilingual Spanish speaking a plus
  • Valid driver’s license and good driving history

Trauma Performance Improvement Specialist Registered Nurse

Position Summary: The trauma PI specialist supports the Trauma program's Performance Improvement and Patient Safety (PIPS) activities, including reviewing of trauma care across the continuum of care, case finding, and audit of medical records. Provides coordination and facilitation of performance improvement activities, including trending and benchmarking of data outcomes. Essential Functions and Responsibilities : Under the general supervision of the Trauma Program Manager and Trauma Medical Director, functions to included, but not limited to: Performs and/or supervises medical record audits related to various trauma quality improvement indicators to identify problems and/or trends in trauma care for referral to providers including but not limited to data for the MTQIP (Michigan Trauma Quality Improvement Program) and TQIP (Trauma Quality Improvement Program. Responsible for providing feedback to referral hospitals/physicians regarding patient outcomes. Will attend 2 of 3 tri annual MTQIP collaborative meetings yearly. Regularly attends Trauma Multidisciplinary Peer Review Physician meeting Prepares chart reviews and summaries for Trauma Peer Review/ morbidity and Mortality. Regularly attends the Trauma Multidisciplinary System meetings. Develops and assists others in developing action plans and ensures event resolution. Assist the Trauma Program Manager and Trauma Registrar. Ensures the accuracy and completeness of trauma registry, including data abstraction, entry, and submission to the database. Reviews trauma patients clinical records including EMS reports, ED, surgery, and inpatient records. Assist to design and provide structured audits and reports of the registries data validation. Make recommendations to improve the process of data collection for the registry. Reports any identified quality issues throughout the trauma program and generated solutions to improve level of performance. Provides coordination and facilitation of performance improvement data collection, analysis, trending and benchmarking of data and outcomes. Identifies and assist with the coordination and implementation of educational activities for health care professionals. Promotes guest relations, anticipates, and responds to needs of others aids in courteous and timely manner, treats others with care and respect maintaining privacy, confidentiality, and dignity. Maintains a professional appearance and a clean, safe work environment. Participates in Community events on behalf of the trauma service as needed. Performs other related job duties as required. Qualifications: Required: Current Registered Nurse license in the State of Michigan Five years of recent nursing experience in surgical care, critical care, and or emergency department pertaining to trauma care. Computer and internet skills and experience including comfort with Windows, MS Office environment and other software application. Preferred: Bachelor of Science in Nursing (BSN) TNCC (Trauma Nurse Core Curriculum), PALS, ENPC, ATCN Previous experience with data abstraction from medical records and or analyzing and interpreting process of care and clinical outcomes. Knowledge, skills, and abilities: Self-directed with excellent communication skills analytical ability to organize, record and abstract data from computer systems and patient EMR to maintain trauma registry and produce various reports. Able to independently devise and verbalize methods for problem resolution Flexible and committed to a team concept. excellent interpersonal skills to be able to interact effectively with physicians and other staff when gathering information.
