Develop, implement, and update individualized Plans of Care. Complete OASIS assessments and clinical documentation accurately and on time. Monitor patient progress and communicate changes with physicians and the interdisciplinary team. Educate patients and caregivers on disease management, medications, treatments, and safety. Coordinate care with physicians, therapists, home health aides, and other healthcare professionals. Supervise Home Health Aides in accordance with state and federal regulations. Promote safe, compassionate, high-quality patient care while helping patients remain safely at home. Qualifications Graduate of an accredited Registered Nursing (RN) program. Current, unrestricted Ohio Registered Nurse license. Current CPR certification. Minimum of one year of nursing experience required. Home Health experience is preferred but not required.
