Summary
The Clinic RN works in collaboration and continuous partnership with clinic patients and their family/caregiver(s),
clinic/hospital/specialty providers and staff, and community resources. The Clinic RN collaboratively assists MLHS
patients applying evidence practice to achieve optimal care.


Job Duties

  • Perform Nurse visits*
    Facilitate process and work flows to augment quality initiatives (meaningful use, PCRS, MN community
    measures)*
  • Assist clinic staff and providers to enable efficient flow of patient care. Tasks to include but not limited to;
    rooming of patients, administering injections, facilitating patient requests by phone and patient portal and
    coordination of outside referrals*
  • Provide medication reconciliation and provide continuity of care by serving as liaison and primary contact for
    retail pharmacy staff; and other care providers*
  • Facilitates medication refills by protocol*
  • Connect patients to relevant community resources and services for patients (e.g. medications, medical
    equipment, home care etc.)*
  • Assist in development and management of anticoagulant program (TBD)
  • Create and promote adherence to a care plan, developed in coordination with the patient, primary care
    provider, and family/caregiver(s) (TBD)
  • Reduce emergency room utilization, hospital readmissions and increase quality of care by coordination of
    chronic care program (TBD)

If assigned a provider:

  • Chart prep; prepares for patient visit through EMR prior to visit
  • Rooming patient which includes height, weight, blood pressure, visual acuity, color vision/hearing screening.
  • Performs and fully completes all documentation in the EMR
  • Administration and documentation of oral and injectable medications and vaccinations
  • Proper storage, handling, and administration of vaccines. Be able to determine vaccines appropriate for patient
    based on past vaccine records, age, and the CDC and MDH vaccine schedules*
  • Complete continuance of patient care by coordinating diagnostic testing and referrals to inside and outside
    providers
  • Provides instructions and education to patients as instructed by providers
  • Sets up prescription refills and route to care team box
  • Review and execute phone notes and forms
  • Completes nurse box and flags
  • EKGs, ear lavages, assisting provider with procedures
  • Restocking of supplies in exam rooms

If assigned front desk:

  • Reception/Clerical task which include but is not limited to: Scheduling appointments, verification of insurance,
    checking in for appointments
  • Completing patient referrals
  • Ordering of supplies and equipment and restocking of supplies in exam rooms
  • Serve on various committees and task forces as assigned.
  • Willing to train and provide a productive learning environment for students, externs, and new staff, including, but not
    limited to Clinical Assistants and Patient Care Technicians.

Required Education and Experience

  • Graduate from an accredited school of nursing with current licensure as a Registered Nurse in the state of
    Minnesota
  • Knowledge of nursing principles, policies, procedures and medical ethics equivalent to years of experience
  • Excellent communication skills across disciplines and at various levels
  • Possession of attention to detail, organization and prioritization skills
  • Basic Computer skills and comprehension
  • Certifications: Basic Life Support required

Preferred Education and Experience

  • Ambulatory care experience
  • Experience in chronic care management
  • Knowledge of community advocacy and community resources

Position Type/Expected Hours of Work
Monday through Friday day hours with occasional Monday evening and Saturday morning rotation. Occasional holidays
and work during inclement weather will be required. This position may require long hours depending upon patient need.

Provides work direction in coordination with the Clinic Director.
