Job Description​

We are hiring an experienced APP for our Mission Farms Primary Care Clinic in Overland Park, KS.

  • This position is accountable for advanced nursing practice in the care of assigned patient populations
  • Responsible for collecting and analyzing client health data
  • Identifies appropriate expected outcomes
  • Develops and implements patient plan of care
  • Provides comprehensive clinical coordination of care and case management
  • Provides consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations
  • Evaluates clients progress in attaining expected outcomes
  • Conducts nursing research and provides education to patient family and nursing staff

Candidates with 2-3 years of advanced practice preferred.

The Psychiatric Nurse Practitioner is responsible for providing comprehensive psychiatric services to incarcerated individuals within a correctional facility and with a collaborating physician.
