Job Description Summary:

The Diabetes Nurse Specialist will work in the outpatient setting and inpatient setting as needed, acting as a member of the patient’s care team that participates in the continuum of care and education of patients with diabetes. Assess patient and pertinent family members’ knowledge of diabetes and develops and implements a patient-centered care plan; monitors and evaluates, as needed, resource utilization, patient outcomes and/or goals, patients’ skills in self-care, and ability/readiness to learn. The position communicates with providers regarding patient needs and ongoing status of care plan. The Diabetes Nurse Specialist provides clinical resource and consultation to staff and demonstrates advanced clinical competence in diabetes care.

Job Requirements, Prerequisites and Essential Functions:

EDUCATION
1. Active RN License, BSN or MSN preferred.
2. Certified Diabetes Educator (CDE) certification required within 90 days of hire.

EXPERIENCE
1. Inpatient and outpatient nursing experience, with clinical expertise in diabetes care.
2. Clinical expertise in diabetes care with insulin pump/CGMS experience.
3. Working knowledge of American Diabetes Association (ADA) recognition for Diabetes Self-Management Education (DSME).
4. Experience in electronic medical records, data management and analysis preferred
5. Experience with individual and group teaching methodologies.

LICENSURE AND CERTIFICATIONS
1. Registered Nurse Required at hire

2. Basic Life Support (BLS) through an American Heart Association (AHA) accredited course required at hire.
2. Certified Diabetes Educator Required within 90 days of hire
Integrative Agitation Management (IAM) Required within 30 days of hire

PREREQUISITE SKILLS
1. Demonstrated knowledge of education theory, principles and methods of adult and/or pediatric education.
2. Demonstrated knowledge of current American Diabetes Association (ADA). recommendation for patient management.
3. Demonstrated teaching and leadership abilities.

Clinical Coordinator/SCU Charge RN (32 hours per week)

Overview This position is responsible for the daily work and work flow within the Surgical Care Unit (SCU). The Clinical Coordinator/SCU Charge RN supervises the nursing staff in coordinating schedules, nursing assignments and patient care in the preoperative and postoperative phases of surgery from preadmission through discharge. Responsibilities Daily staffing: Assigns nursing staff according to caseload, types of surgery cases, and staffing needs in accordance with Title 22. Knows the strengths of each staff member and works to provide ongoing training and mentoring as needed to bring all staff to similar levels of expertise in both the pre and post op care of surgery patients. Overview The Case Manager leads care coordination throughout hospital levels of care. In this role the Case Manager completes assessments and transition care planning that reflects the wishes and needs of the patient and their family. This position also assists physicians and finance with the management of resource utilization through the use of medical necessity criteria. Responsibilities Completes medical necessity criteria for both inpatient and outpatient visits, documenting the reasons when the patient status is not supported in the database and reporting discrepancies during weekday morning huddles. Completes assessments and discharge plans for inpatients. Provides consultation for outpatient and observation patient stays when requested. Assists with discharge planning for these patients as needed. Builds provider networks and community resources and provides updates to the clinical team. Overview The Emergency Nurse is responsible for high quality individualized patient care in the Emergency Department requiring a broad range of knowledge and expertise in the treatment of patients experiencing medical emergencies ranging from low acuity (earaches) to full blown resuscitative encounters (acute stroke, cardiac arrest).
