Job Summary

As supervised and delegated by a Registered Nurse, provides direct patient care in the clinical setting.

Essential Functions

  • Interacts professionally with patients/families; treats patients and their families with respect and dignity.
  • Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
  • Effectively and appropriately communicate with patients, physicians, family members and co-workers, demonstrating customer service excellence through application of positive language principles, appropriate response and use of constructive problem-solving techniques which foster positive outcomes.
  • Provides direct patient care, consults with nursing staff/providers as required and adjusts nursing care processes as indicated to ensure optimal patient care. Ability to adequately assess and reassess pain.
  • Utilizes appropriate pain management techniques.
  • Educates the patient and family regarding pain management.
  • Demonstrates knowledge of lab requirements, requisitions.
  • Collects a baseline summary of a patient’s medical to include, check and record a patient's body temperature, pulse, respiration weight and blood pressure.
  • Maintains accurate and continued documentation appropriately for all assessments, treatments, and evaluations performed.
  • Documentation meets current standards and policies.
  • Decisions made reflect knowledge of facts, knowledge of diseases/surgical conditions, care required and good judgment.
  • Demonstrates knowledge of the principles of growth and development over patient life span and the skills necessary to provide age-appropriate care to the patient population served.
  • Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group.
  • Apply principles of sterile techniques and infection control procedures.
  • Follows the six (6) medication rights and reduces the potential for medication errors.
  • Maintains current knowledge of medications and IV fluids and their correct administration based on age of the patient and his/her clinical condition.
  • Assists with establishment of plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.
  • Identifies and documents goals, anticipated progress and plans for reevaluation.
  • Assists other nursing staff with the delivery of nursing care.
  • Consults other providers as appropriate to collaborate in patient care and performance improvement activities.
  • Coordinate patient services with referring physician / facility.
  • Performs other reasonably related job duties as assigned or requested, within scope of practice.

Qualifications

Education

  • Graduate of an accredited program for licensed vocational/practical nurses.

Licenses/Certifications

  • Licensed Practical/Vocational Nurse License.
  • BLS Certification required.
  • Intravenous Therapy and Blood Certifications preferred.
  • ACLS may be required in certain care areas

Experience

  • Some experience preferred.

Purpose of Position: Delivers patient care through the nursing process of assessment, planning, implementation, and evaluation. Administers and monitors patient responses to medication. Directs and guides patient care and education while maintaining standards of professional nursing within the realm of MACC and CCHC Policy. Qualifications LPN - EDUCATION/EXPERIENCE/TRAINING: Ability to read, write and communicate in English Graduate of an accredited Practical Nursing program Currently licensed as a Licensed Practical Nurse in the state of Massachusetts Basic Life Support Certification for Health Care Providers Strong interpersonal, customer service, and time management skills Experience with current office technology and proficiency with computer office applications (Electronic Medical/Health Records, MS Office, etc.) A minimum of two years experience in a clinical setting preferred Rn - Education/Experience/Training Ability to read, write and communicate in English Graduate of an accredited school of nursing Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts Basic Life Support Certification for Health Care Providers Strong interpersonal, customer service, and time management skills Experience with current office technology and proficiency with computer office applications (Electronic Medical/Health Records, MS Office, etc.) A minimum of two years experience in a clinical setting preferred Schedule Details: 16 hrs. per week, Rot.
