What You Bring

  • Active RN license
  • Graduate of an accredited nursing program (BSN preferred)
  • Home health or related behavioral health clinical experience
  • Strong communication, organization, and critical thinking skills
  • Current CPR certification
  • Valid driver’s license and reliable transportation

The Adolescent and Young Adult (AYA) Nurse Navigator functions as a clinical expert and care coordinator for AYA Program of the Holden Comprehensive Cancer Center (HCCC). The AYA Nurse Navigator functions as a clinical liaison between the primary healthcare team and patients/families with attention to the unique experiences to AYA cancer patients including psychosocial care, onco-fertility and clinical trial enrollments. They will provide seamless patient-centered navigation through the continuum of care from initial diagnosis until transition into survivorship or end-of-life care Position Responsibilities Serves as a clinical expert and resource to AYA patients with a cancer diagnosis ages 18 to 39. Meets with patient at enrollment to AYA program to make a face-to-face introduction and provide navigation related education.
