We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity. Preferred Qualifications Role Purpose The Certified Nurse Assistant is responsible for providing direct patient/resident care and assistance in accordance with all applicable policies, procedures, regulations and nursing standards of care. Responsibilities Obtains vital signs and other measurements as directed and documents findings promptly. Meets patient hygiene, comfort, nutrition, and hydration needs and documents as required. Recognizes and reports abnormal findings or changes in patient status to the nurse throughout entire shift. Implements care under the supervision of the nurse.or Primary Case Manager. In Home Health, this can be done with a RN/PT/OT or Speech Pathologist.
