Schedule: 4 - 10 hr shifts, Saturday and Sunday and 2 week days

What You'll Do as a Hospice RN / RN Case Manager:

  • Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.

  • Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home.

  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.

  • Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans.

  • Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance.

  • Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance.

  • Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions.

  • Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards.

  • Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient.

  • Help patients and families understand and navigate topics such as: 

  • Medication administration 

  • Hospice philosophy and services 

  • Symptom and pain management 

  • End-of-life processes and expectations

About You

Qualifications – What You’ll Bring:

  • Active RN license in the state of employment (or eligible to obtain).

  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.

  • A genuine hospice heart: compassionate, empathetic, and patient-centered.

  • Comfortable providing care in diverse settings, including private homes and facilities.

  • Strong communication and critical thinking skills in emotionally sensitive situations.

  • Valid driver’s license, auto insurance, and reliable transportation.

  • CPR certification required.

Preferred Experience (Not Required):

  • Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)

  • Admissions RN, wound care, float pool, travel nursing

  • Experience with terminally ill patients or serious illness support

NURSING PROCESS: ASSESSMENT and DIAGNOSIS -- Collects and documents comprehensive data pertinent to the patient's health or the situation. Assessments are thorough, timely and accurate and include but are not limited to the physiological, socio-cultural, safety and spiritual needs of patients (including significant other). Demonstrates ability to use critical thinking skills to analyze assessment data to determine the pertinent diagnosis or issues. Responds appropriately to changing needs of patients, including response to emergency/life threatening situations. OUTCOMES IDENTIFICATION / PLANNING -- Identifies expected outcomes for a plan individualized to the patient or situation. Develops and documents an individualized plan of care which incorporates the patient's significant other as appropriate and that prescribes strategies and alternatives to attain expected outcomes. Identifies initial and ongoing discharge planning needs. Demonstrates the ability to identify, evaluate and document the patient's response to care given. Coordinates care across the continuum to meet the identified and anticipated needs of the patient and his/her support network. IMPLEMENTATION -- Functions as advocate for patient/significant other. Demonstrates skills needed to implement the identified plan of care, incorporating elements of Standards of Professional Performance (as described in Essential Functions #1) as appropriate. Demonstrates ability to prioritize and adjust priorities based on patient/unit needs. EVALUATION -- Demonstrates ability to evaluate and document patient's progress towards attainment of desired outcomes. PATIENT EDUCATION: Demonstrates ability to optimize patient learning through evaluating a patient's learning abilities and carrying out the education of the patient. Requirements: -Graduate of accredited Registered Nursing Program required. -Bachelor's of Science Nursing (BSN) preferred -Current Michigan RN License (or completed by end of orientation) required -Current BLS for the Healthcare Provider (or completed within 60 days of hire) required -For ICU/TU, Cardiac Cath Lab, Emergency, PACU, Surgery Center, Endoscopy, Spine/Orthopedics, Float Pool: ACLS Certification or completed within one year of hire. -For Emergency, 5 West, PACU, Surgery Center: PALS certification or completed within one year of hire. -For Birth Center: NRP Certification or completed within one year of hire. -Certification in specialty area (CCRN, CRN, CNOR, CEN, etc) preferred -Recent hospital experience and/or previous nursing practice experience preferred -2 years prior RN experience in desired specialty area preferred -Prior experience with electronic patient documentation system preferred -Prior experience with technology common to specialty area (i.e., cardiac monitors, IV pumps) preferred -Bilingual preferred #EDRN123 Holland Hospital is an Equal Opportunity Employer, please see our EEO policy
