Responsibilities

Obtains temperature, pulse, blood pressure and respiration.
Maintains a clean, safe, and healthy environment.
Assists patient with getting in and out of bed, bathing, hygiene, and toileting.
Prepares and provides and/or assists with meals.
Performs light homemaking and other environmental services.
Observes, reports, and documents any changes in patient status.
Understands basic elements of body functioning and reports changes in patient's body functions as indicated.
Recognizes emergency situations and implements appropriate emergency procedures per care center policy.
Follows care center procedures regarding infection prevention and control, handling of hazardous waste and safety measures.
Follows specific written instructions for personal care in accordance with the plan of care.
Assists patient with prescribed exercises as instructed per nurse or therapist.
Performs other duties as assigned.

Qualifications

High school diploma or equivalent.
Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
CNA experience within the last two years. If it has been more than two years, you must complete another training and competency program before providing services.
Must be able to successfully pass the competency program of the care center.
Current CPR certification.

The Home Health Aide is a certified individual who provides direct patient care (personal care) as designated by the Registered Nurse Case Manager, Physical Therapist, Occupational Therapist, or Speech Therapist. Moving or assisting with transferring of patients may involve lifting of up to 50 lbs., individually, or 100+ lbs. as part of a team-lift, although clients must be weight-bearing or bed-bound as determined by the nurse. The patient's Care Plan specifies a one-person, two-person or mechanical-assist lift. Home Health Aides are regularly required to stand and talk or hear, and frequently required to walk; they use hands to finger, handle, or feel; and they reach forward with hands and arms. They are occasionally required to sit, stoop, kneel, or crouch. Home Health Aides must be able to drive a motor vehicle or take public transportation and would sit during such times. Home Health Aides must frequently lift and/or move items, materials and supplies up to 10 lbs. and occasionally lift or move items or materials up to 30 lbs. Standing and walking are constant during assisting. Home Health Aides are often seated when completing paperwork, for about 15 minutes at a time, sitting on average for a total of 1 hour over an 8-hour shift.
