Hours: Tuesday - Friday days, noon-10pm

The Admissions Nurse, will be responsible for but not limited to:

  1. Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission.
  2. Ability to outline the patient/family referral and admission.
  3. Uses assessment date to determine care plan needs and initiates the plan of care.
  4. Articulates hospice philosophy, goals and objectives.
  5. Communicates with other members of the clinical team to coordinate the care plan.
  6. Markets to acute hospitals in the area.

Education:

Associate's Degree in Nursing required, Bachelor's Degree preferred

Professional Experience:

  • 2-4 years of related Hospice experience
  • Licensed as a Registered Nurse within the state of operations

Strong communication skills. Valid driver's license & reliable transportation. Must be available to travel locally within assigned territory.
