Licensed Practical Nurse (LPN) Job Summary:

The Licensed Practical Nurse (LPN) is responsible for the management of an assigned unit on a specific tour of duty.

 

Licensed Practical Nurse (LPN) Job Responsibilities:

  • Know and support the philosophy, goal and objectives of the Nursing Department.

  • Supervise Nursing Assistants and Staff Assistants.

  • Assign resident care responsibilities to nursing personnel based on residents' needs, available staff and unit needs.

  • Collaborate, communicate and cooperate as appropriate with other health care providers to insure quality care, including transcribing physician's orders according to facility procedures.

  • Continually observe/monitor and evaluate quality, quantity and timeliness of nursing care.

  • Deliver treatment modalities according to physicians orders

  • Implement and update care plans as appropriate

  • Ensure documentation is appropriate and accurate at all times

  • Assess need for modification of medical regimen and/or nursing care plan and ensure the clinical records are properly maintained.

  • Direct the delivery of care using sound good judgment while applying the highest standards of care and within the nurse practice act.

  • Provide for and assesses the most complex resident needs to ensure appropriate care

  • Teach appropriate nursing techniques to residents, staff and/or family.

  • Administer medications and treatments according to physician’s orders and established policy and procedures.

  • Report falls, unusual occurrences and significant changes in resident's condition to the supervisor, physician, caseworker and responsible party according to facility policy and procedures.

  • Initiates and when appropriate completes investigations of resident grievances, incidents, elopements, falls, abuse allegations, and any other unusual incidents that require an investigation.

  • Document according to requirements of facility and maintain strict confidentiality.

  • Give appropriate report in a professional manner to the nurses on next shift according to requirements of facility.

  • Ensure established inventory standards and proper maintenance of equipment is complete.

  • Seek consultation as needed from supervisor.

  • Assist the Unit Manager in conducting team conferences to assist in planning nursing care, problem-solving techniques for unit problems and evaluating staff's involvement and attitude.

  • Attend continuing educational programs relating to responsibilities of the position and professional growth.

  • Act as role model for nursing assistants, licensed staff and other personnel.

  • Maintain and enforce department and facility procedures, fire prevention and safety standards

  • Comply with Health Insurance Portability and Accountability Act (HIPAA) regulations.

 

Licensed Practical Nurse (LPN) Requirements:

  • Currently Licensed Practical Nurse (LPN) license in the State of Pennsylvania.

  • Must be knowledgeable of nursing, medical practices and procedures.

  • Must be knowledgeable of State and Federal Regulations for Long Term Care.
