Monitors and charts patients, collects sample, and performs other tasks as assigned, all under the direction of registered nurses and doctors. Cardiovascular disorders cared for on 3 South include: MI-following the acute stay in the HVICU, Heart Failure; Pulmonary Edema; Arrhythmias; Post-interventional Radiology patients; Post-procedure Angioplasty; Valvular Heart Disease. The staff cares for Thoracic Surgery patients undergoing various thoracic surgical procedures such as lobectomy, esophagectomy, sleeve resection/chest wall resection, VATS, lung volume reduction surgery and endobronchial valves. This position will support the geriatric oncology population and the palliative care practice. This is a part-time, day shift opportunity. Summary: The Nurse Navigator functions as a resource to a multidisciplinary team of specialists as an advocate, interpreter, educator and counselor for the oncology patient, their family, and referring public. The patient navigator is responsible for ensuring patients with an oncology diagnosis receive quality and comprehensive coordinated services. The patient navigator will coordinate care throughout the care continuum in collaboration with the multidisciplinary team. The scope of this work includes facilitating dynamic scheduling for patients, triage services and patients as needed, coordination of care, facilitation of clinical research, integration of clinical activities across various specialties in cancer care.
