Key Responsibilities:

  • Administer medications and treatments in accordance with physician orders and facility protocols.

  • Create and implement care plans that address residents’ individual needs.

  • Provide skilled nursing care including wound care, vital sign monitoring, and reporting changes in condition.

  • Maintain accurate and timely documentation.

  • Ensure residents' rights and privacy are respected in accordance with HIPAA and facility policies.

Qualifications:

  • Current, valid LPN license in the State of New Jersey.

  • Skilled Nursing Facility experience preferred but not required.
