Registered Nurse – Intensive Mobile Treatment Team (Full Time)
About the Program
Intensive Mobile Treatment (IMT) is a trauma-informed, community-based behavioral health program that provides treatment, rehabilitation, and support services to individuals who have not been successfully served by traditional systems. IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings.
Position Overview
The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals.
Key Responsibilities
· Conduct psychiatric and physical health assessments for program participants.
· Develop and coordinate care plans with the multidisciplinary treatment team.
· Monitor participant health status and modify treatment plans as needed.
· Administer medications, injections, vaccines, and perform phlebotomy as ordered.
· Monitor vital signs and medication side effects and communicate findings to the treatment team.
· Coordinate medical appointments and collaborate with psychiatrists and community physicians.
· Provide treatment, rehabilitation, and recovery support services.
· Advocate for participants and assist them in accessing community resources and services.
· Maintain accurate clinical documentation, progress notes, and medical records.
· Participate in team meetings, supervision, and required trainings.
· Provide crisis intervention services on a rotating basis.
Qualifications
· Currently licensed as a Registered Professional Nurse (RN) by the New York State Education Department.
· Minimum two (2) years of experience working as a Registered Nurse.
· Minimum two (2) years of experience working with individuals with mental health conditions in a health or social services setting.
· Strong written, verbal, and interpersonal communication skills.
· Ability to assess situations, synthesize information, and make appropriate clinical decisions.
· Eligible for full and unconditional participation in Medicaid and Medicare programs.
· Computer proficiency including MS Word, Excel, PowerPoint, and electronic health systems.
· Comfort providing services in community settings such as shelters, public spaces, and outreach environments.
· Ability to travel within the community, including use of public transportation.
· Bilingual Spanish preferred.
· Must be fingerprinted and cleared by the New York State Justice Center.
