School LPN Nurse Needed in the Bronx 2 days per week, 9am-5pm! Scope of Services Consultant will coordinate and oversee the health care services across the 2 sites. This includes: -Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. This also includes managing all health and nutrition components of the programs in accordance with federal and state regulations, implementing the medication policy including administration of medication, measuring blood pressure, blood sugar and monitor records to ensure compliance with regulatory standards, and function as part of a cohesive team that develops and monitors program operations. Conducting Physical Assessments: -Assist all two center base sites with health related information for each child and review all medical information and charts. -Document visits at all sites for data collection in accordance with all guidelines for health- related child outcomes. -Review child physicals upon enrollment and on a regular basis thereafter to ensure appropriate updates and sustaining of ongoing physical updates. -Ensure all physicals have been completed within the last year. -Ensure the child is up to date with vaccinations. -Provide guidance for the completion of Child Health Care Plans and monitor every six months for update compliance, renewals, or changes in medication and to update doctor/parent permission to continue administering. -Review of any medications that are related to allergies or specific medical care instructions and note this information in the children's file. Professional Responsibilities: -Complies with medical, nursing, and educational ethics. -Exhibit strong organizational and administrative skills to implement and promote the health program. -Case management, health intervention and interdisciplinary collaborating skills. -Awareness of community resources and referral information. -Advanced skills in interpersonal communication and health counseling. -Implement advanced nursing protocols on pediatrics and adolescent medicine . We are seeking a Registered Nurse (RN) for their Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The Registered Nurse (RN) provides comprehensive nursing services across the CRTP, ICM, and CTI. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Salary : $95,000 per year plus benefits. Schedule: Monday - Friday 9am-5pm Essential Responsibilities (including but are not limited to): Clinical Care -Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. -Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. -Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. -Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. -Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. -Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. -Monitor and manage medication storage and disposal procedures in accordance with federal and state law. -Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status. -In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. -Order and maintain necessary first aid and medical supplies as needed. -24/7 availability via phone for consultation and basic direct care. Case Management & Transitional Support -Partner with ICM teams to address both medical and behavioral health needs in service planning. -Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness. -Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support. -Coordinate follow-up care with primary care providers, specialists, and community-based resources. Collaboration & Training -Serve as the clinical liaison among providers, families, and program staff. -Participate in interdisciplinary treatment planning and case conferences. -Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response. -Maintain timely and accurate documentation in compliance with OPWDD, OMH, and DOH regulations. Qualifications -Current New York State RN license (unrestricted). -Minimum 2 years of nursing experience; background in IDD, behavioral health, crisis care, or community health preferred. -Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements. -Strong clinical assessment, crisis intervention, and care coordination skills. -Excellent communication, organizational, and documentation skills. -Ability to work independently while thriving in a team-based environment. -Valid driver's license preferred (travel to multiple program sites may be required). Certified Nursing Assistant (CNA) needed in Brooklyn Well-staffed Nursing and Rehabilitation Center 3pm-11pm JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques. Cooperation Works well with co-workers and supervisors. Maintains appropriate relationship with residents, families and peers. Gives assistance to other departments: i.e. recreation, when necessary.
