Maintains a safe and clean working environment Maintains resident confidentiality by monitoring confidential information processing. Evaluates work performance of nursing staff. Provides disciplinary action and participates in grievances/arbitration. May immediately suspend and/or require staff to clock out and leave the facility for such unacceptable actions as drunkenness, fighting, patient abuse, refusing an assignment, insubordination, etc. Assists in the recruitment, selection, promotion, and termination of employment of nursing personnel. Assures adequate staffing for all units calling replacement staff as necessary. Completes investigation sheets on all accident reports and submits to ADNS. Monitors for adequate supply of oxygen and for proper storage, ordering additional oxygen as needed. Monitors emergency supplies and emergency drug boxes assuring readiness for emergencies. Registered Nurse – Intensive Mobile Treatment Team (Full-Time) About the Program Intensive Mobile Treatment (IMT) is a trauma-informed, community-based behavioral health program that provides treatment, rehabilitation, and support services to individuals who have not been successfully served by traditional systems. IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings. Position Overview The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals. Key Responsibilities • Conduct psychiatric and physical health assessments for program participants. • Develop and coordinate care plans with the multidisciplinary treatment team. • Monitor participant health status and modify treatment plans as needed. • Administer medications, injections, vaccines, and perform phlebotomy as ordered. • Monitor vital signs and medication side effects and communicate findings to the treatment team. • Coordinate medical appointments and collaborate with psychiatrists and community physicians. • Provide treatment, rehabilitation, and recovery support services. • Advocate for participants and assist them in accessing community resources and services. • Maintain accurate clinical documentation, progress notes, and medical records. • Participate in team meetings, supervision, and required trainings. • Provide crisis intervention services on a rotating basis. The Registered Nurse (RN) provides comprehensive nursing services across the Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. ESSENTIAL RESPONSIBILITIES Clinical Care Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. Monitor and manage medication storage and disposal procedures in accordance with federal and state law. Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status. In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. Case Management & Transitional Support Partner with ICM teams to address both medical and behavioral health needs in service planning. Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness. Coordinate follow-up care with primary care providers, specialists, and community-based resources. Collaboration & Training Serve as the clinical liaison among providers, families, and program staff.
