The Brook at High Falls Nursing and Rehabilitation Center is currently seeking a Full-Time LPN to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by the certified nursing assistants (CNAs). Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility and as may be required by the Director of Nursing or Nurse Manager to ensure that the highest degree of quality care is maintained at all times.

EDUCATION, EXPERIENCE AND QUALIFICATIONS:

Must possess a current, unencumbered, active license to practice as an LPN in NYS.

DUTIES AND RESPONSIBILITIES:

• Maintain the safety to the residents at all times.

• Administer medications as directed by the physician orders.

• Change dressings as directed by the physician orders and report all changes is wounds to RN Nurse Manager or Supervisor. Assure physician orders are updated.

Maintain the Daily 24-hour Report and submit to Supervisor as required.

• Direct CNAs in the completion of their duties.

• Assist CNAs to help meet the needs of the residents when available (i.e. toileting, call lights, etc.)

• Maintain a cohesive understanding with families/caregivers of the resident.

• Complete accident/incident reports as necessary and chart all reports involving residents.

• Report all complaints and grievances made by the resident to the appropriate source.

• Perform any other pertinent duties as assigned by my supervisor.

PERSONAL REQUIREMENTS AND WORKER TRAITS:

• Must possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public.

• Must be able to cope with mental and emotional stress of the position.

• Must be willing to work as a team player.

• Must be able to relate information concerning a resident's condition.

• Must be able to relate to and work with the ill, disabled, elderly, and emotionally upset people within the facility.

Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Licensed Practical Nurse is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The LPN will work on a Full-Time basis and using a hybrid work model and will cover services in the following counties: Ontario, Seneca, Yates, Steuben, Schuyler, Tompkins and Chemung counties. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.
