LPN Unit Manager

LPN Unit Manager Position Summary The Brook at High Falls is seeking an LPN Unit Manager. The role is responsible for overseeing the daily clinical operations of an assigned nursing unit to ensure the delivery of high-quality resident care. This role provides leadership and support to nursing staff, promotes regulatory compliance, and collaborates with the interdisciplinary team to achieve positive resident outcomes. Key Responsibilities Supervise and support nursing staff on the assigned unit. Coordinate resident care and monitor clinical outcomes. Ensure compliance with state, federal, and facility regulations. Conduct rounds and address resident, family, and staff concerns. Assist with care planning, admissions, discharges, and physician communications. Monitor documentation accuracy and completion. Participate in quality assurance and performance improvement activities. Promote a positive, resident-centered care environment. RN Unit Manager

$40 - $45 / hour
The Brook at High Falls is looking for an experience Nurse Manager. Provide leadership and guidance for other professional and ancillary staff. The Nurse Manager is responsible to assure the optimal level quality of care is provided to the residents. EDUCATION, EXPERIENCE AND QUALIFICATIONS : Must be a Registered Professional Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current New York State registration and have should have previous geriatric and / or supervisory experience in NYS. Must be CPR certified. DUTIES AND RESPONSIBILITIES: • Work by prearranged assignment, but must be able to accept changes and adapt to added assignments as the need arises. • Assume responsibilities for and direct activities relating to health and nursing care of all residents at Healthcare Nursing Center. • Make rounds frequently to check all residents; check with residents mentioned in report to evaluate their condition, and participate in direct services whenever the need arises. • Accurate reporting and documentation of resident status and care needs information. • Evaluate and recommend improvements needed to give quality care to residents. • Assist resident's attending physician during visits/rounds. • Report to physicians residents' condition changes, etc., as indicated. • Record telephone orders on physician order sheets and that nothing/follow through occurs by Charge Nurse. • See that medications are administered and recorded according to the Pharmaceutical Policies and Procedures and that narcotics are accurately accounted for. • Assist with updating care plans as indicated. LPN (Part-Time)

$28.30 / hour
The Brook at High Falls Nursing and Rehabilitation Center is currently seeking a Part-Time LPN to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by the certified nursing assistants (CNAs). Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility and as may be required by the Director of Nursing or Nurse Manager to ensure that the highest degree of quality care is maintained at all times. EDUCATION, EXPERIENCE AND QUALIFICATIONS: Must possess a current, unencumbered, active license to practice as an LPN in NYS. DUTIES AND RESPONSIBILITIES: • Maintain the safety to the residents at all times. • Administer medications as directed by the physician orders. • Change dressings as directed by the physician orders and report all changes is wounds to RN Nurse Manager or Supervisor. Assure physician orders are updated. Maintain the Daily 24-hour Report and submit to Supervisor as required. • Direct CNAs in the completion of their duties. • Assist CNAs to help meet the needs of the residents when available (i.e. toileting, call lights, etc.) • Maintain a cohesive understanding with families/caregivers of the resident. • Complete accident/incident reports as necessary and chart all reports involving residents. • Report all complaints and grievances made by the resident to the appropriate source. • Perform any other pertinent duties as assigned by my supervisor. PERSONAL REQUIREMENTS AND WORKER TRAITS: • Must possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public. • Must be able to cope with mental and emotional stress of the position. • Must be willing to work as a team player. • Must be able to relate information concerning a resident's condition. • Must be able to relate to and work with the ill, disabled, elderly, and emotionally upset people within the facility. VOCATIONAL DEVELOPMENT: Attends in-service programs and staff development programs as provided by the facility to maintain compliance with the Department of Health and other regulatory agencies, as well as Nursing Home policies and procedures.
