Analyzes patient information, including medical history, diagnostic results, and current condition, to develop individualized nursing care plans. Prepares nursing processes, such as medication administration and treatments to provide comprehensive and effective patient care. Reconciles medical records to ensure accuracy and compliance with regulatory standards and organizational policies. Evaluates patient response to interventions and identifies any additional interventions needed to meet defined goals of care. Collaborates with interdisciplinary care team to ensure effective care planning. Performs other duties as assigned. Working under minimal supervision, this role serves as a resource and correspondence hub for clinical and administrative communication, applying knowledge to develop, review, and prepare essential operational and clinical protocols. This role seeks guidance from senior management on complex cases. Essential Job Function Reviews patient cases, medical histories, and current treatment plans to effectively navigate patients through their healthcare journey. Develops care plans, in coordination with various healthcare providers and departments to ensure a streamlined and supportive experience for patients. Prepares and maintains records of patient interactions, care plan developments, and navigational interventions to ensure consistent and coordinated care. Applies knowledge of healthcare systems, resources, and patient needs to guide patients through their medical journey, ensuring access to necessary services and support. Examines navigational strategies and interventions, to ensure they are patient-centric. Corresponds consistently with healthcare teams and patients, ensuring clear communication and continuity of care, while also serving as a key resource for the company in enhancing patient navigation services, and occasionally seeks guidance from senior management for complex navigational challenges.
