RN Clinical Case Manager Job Description:

Develop and implement individualized care plans, ensuring patient needs are met and goals are achieved.
Conduct comprehensive patient assessments, including vital signs and medical records review.
Collaborate with healthcare providers, including physicians and social workers, to ensure comprehensive patient care.
Perform utilization management duties, including reviewing medical records and assessing patient eligibility for services.
Provide patient education and support to promote behavioral health and self-care.
Monitor and report patient outcomes, identifying areas for improvement and developing strategies for improvement.
Maintain accurate and detailed patient records, adhering to medical records and HIPAA regulations.

RN Clinical Case Manager Requirements:

Private Duty Homecare experience
RN Registered Nurse NYS license
Managment experience a plus
Proficiency in medical terminology and utilization management principles.
Ability to assess and manage patient care, including vital signs and medical records review.
Excellent communication and collaboration skills.
Strong analytical and problem-solving skills.
Ability to work effectively in a fast-paced environment and prioritize tasks.
Knowledge of medical records and HIPAA regulations.
