Centers Healthcare is hiring a Licensed Practical Nurse (LPN)

A well-established skilled nursing facility in the Bronx is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team.

Responsibilities:

  • Gather and document essential information for new resident admissions.
  • Monitor and record residents’ vitals and overall health status.
  • Administer medications, injections, and treatments as prescribed.
  • Provide wound care and assist with bedsores.
  • Supervise Certified Nursing Assistants (CNAs) as needed.
  • Assist residents with personal hygiene and daily care.
  • Track and report residents’ food and fluid intake/output.

Requirements:

  • Active and valid LPN license.
  • Must be in good standing with the State Registry.
  • Strong teamwork and communication skills.
