Job Summary

Staten Island Care Center is seeking a compassionate and dependable Home Health Aide (HHA) to join our team on a PRN basis. In this role, you will assist residents with transportation to and from medical appointments, ensure their safety during outings, and provide a supportive and caring presence throughout the day.

Key Responsibilities

Escort residents to and from clinic appointments and dialysis sessions.
Monitor residents to prevent falls or altercations, intervening promptly as needed.
Assist residents in preparing for appointments and ensure snacks are provided during out-on-pass periods.
Report all incidents and accidents promptly to the Clinical Coordinator or ADNS.
Represent the facility in a professional and courteous manner, upholding all policies and procedures both on and off-site.

Minimum Qualifications

Current New York State Home Health Aide (HHA) certification required.
Strong communication skills and a dependable, flexible attitude.
Compassionate, patient, and committed to providing excellent resident care.
