Job Summary:

We are seeking a dedicated and compassionate Admissions Nurse (RN) to join our team at Robinson Terrace Rehabilitation and Nursing Center, located in Stamford, NY. As a key member of our interdisciplinary team, you will be responsible for overseeing and coordinating the admissions process for new residents and hospital returns. This is a full-time role with a weekday-only schedule, providing a strong work-life balance. The ideal candidate will have the ability to work independently and as part of a collaborative team to ensure the highest level of patient care.

Job Details:

  • Schedule: Monday–Friday | 11:00am–7:00pm

  • Pay Rate: $45.00–$50.00 per hour (negotiable based on experience)

Job Responsibilities:

  • Conduct comprehensive admission assessments for new residents and hospital returns

  • Initiate individualized care plans and update as conditions change

  • Review PRI assessments and coordinate all components of the admissions process

  • Participate in discharge planning and care transitions

  • Ensure accurate, timely documentation in compliance with facility policies

  • Perform chart audits to maintain regulatory and clinical standards

  • Prepare MDS assessments, care plans, and required admission documentation

  • Maintain HIPAA compliance and confidentiality at all times

  • Support nursing operations under the direction of the Director of Nursing

  • Supervise and guide nursing staff as needed during the admissions process

  • Serve as a clinical resource for interdisciplinary team members

Requirements:

  • Active, unrestricted New York State RN license

  • Long-term care experience preferred

  • Strong clinical assessment and independent decision-making skills

  • Supervisory or leadership experience preferred

  • Proficient in Microsoft Word, Excel, and Outlook

  • Excellent communication, organization, and interpersonal skills
