Position Summary:
Aventura Skilled Nursing Facility is seeking an experienced MDS Coordinator (LPN/RN) to lead resident assessments and ensure accurate, timely MDS submissions. This key role supports quality care, regulatory compliance, and interdisciplinary care planning.

Qualifications:

  • Active LPN or RN  license (in good standing)
  • Minimum 2 years of MDS experience in long-term care
  • Strong knowledge of RAI process, PDPM, and reimbursement
  • Detail-oriented with excellent organization and communication skills
  • Team player with ability to work independently

Key Responsibilities:

  • Complete and submit accurate MDS assessments (MDS 3.0)
  • Ensure compliance with CMS and state regulations
  • Lead care plan meetings and coordinate IDT collaboration
  • Oversee CAAs and individualized care plans
  • Audit documentation to support coding accuracy
  • Track assessment schedules and maintain compliance
  • Support audits, surveys, and quality improvement initiatives
