Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

  • 5pm-8am
  • Weekends and holidays as assigned
  • On-call/after-hours coverage

Key Responsibilities

  • Respond to after-hours patient calls and urgent care needs
  • Perform skilled nursing assessments and symptom management
  • Provide emotional support and education to patients and families
  • Implement and update patient plans of care as needed
  • Communicate patient condition changes to the interdisciplinary team
  • Collaborate with physicians and caregivers regarding patient needs
  • Complete accurate and timely clinical documentation
  • Uphold hospice philosophy, policies, and regulatory standards

Qualifications & Requirements

  • Current RN or LVN license in good standing
  • Hospice or home health experience preferred
  • Strong clinical judgment and ability to work independently
  • Excellent communication and problem-solving skills
  • Compassionate presence in emotionally sensitive situations
  • Valid driver’s license
  • Reliable, insured vehicle in good working condition (per state and organizational requirements)
