PRIMARY FUNCTION The Admissions Registered Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the admission of appropriate patients. JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE 1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Admissions Registered Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge. 4. In the event the location does not have an Admissions Manager, the Admissions Registered Nurse will receive direction from the Patient Care Administrator or program designee QUALIFICATIONS FOR THE ADMISSIONS REGISTERED NURSE 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Anchor Health is looking for a Licensed Vocational Nurse (LVN) to join our After Hours-On Call team! Position Pay: $32-$38/hr. Days and times worked: Weekend; 8am-5pm On Call Why work at Anchor? Paid for the entire shift, not per visit done This is a part-time position (with the opportunity to expand into a FT position in the future and will be guaranteed 8 hours per shift regardless if you go out for a nursing visit or not) Mileage reimbursement to/from visits Patient-focused care Our mission is to honor and promote dignity and quality of life for our patients and their loved ones during end-of-life care You will have a supportive team there for you at all times The non-negotiables: Must have a valid Licensed Vocational/Practical Nursing license within the state of California Must have at least one year of experience as an LVN/LPN Hospice experience preferred, but not required Must have evidence of CPR training Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient’s location. What you will be doing: Covering our weekend (Friday & Saturday) evenings 5p-8a as an LVN runner. This individual will be on-call for any symptom management/PRN visits that come up after hours and are assigned by our Triage RN. You will not need to answer any calls directly, our Triage RNs are available 24/7 and they will be reaching out to you in case of a visit need. This is strictly a field position. Documentation is done through Homecare Homebase (HCHB – PointCare); all timesheets and mileage are reimbursed directly through our EMR system.
