The Position

As we continue to grow, we are looking for full-time nurse practitioners to provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting. Our nurse practitioner positions can include:

VITAE - Nurse Practitioner Program The Position As we continue to grow, we are looking for “SNF-ists,” full-time psychiatric nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting and bring quality mental health services to those communities. What You Will Do As the psychiatric nurse practitioner, you will act as a Skilled Nursing Facility’s leading provider of psychiatric services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents. You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. As a psychiatric nurse practitioner, you will provide types of care, including but not limited to: Evaluation of new-admission visits Coordination of discharge as needed Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition Other Duties & Responsibilities Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease Prescribe all necessary medications and treatments Facilitate referrals to other healthcare professionals and medical facilities Communicate with attending physicians and/or specialists regarding patient care Proficiency with documentation and CPT coding Working with Vitae will provide you with: · A healthy work-life balance with flexibility.
