Clinical Manager (Registered Nurse) – Home Health & Group Homes

Position Summary

The Clinical Manager is responsible for overseeing clinical operations across home health services and affiliated group home locations. This role requires a highly experienced Registered Nurse with strong knowledge of OASIS, care planning, and regulatory compliance. The Clinical Manager serves as the primary clinical point of contact for staff, patients, families, and physicians, ensuring high-quality, coordinated care delivery.

Requirements

  • Active Registered Nurse (RN) license (Florida)

  • Minimum of 3+ years of home health experience (required)

  • Strong experience with OASIS documentation (required)

  • Experience with medically complex patients

  • Ability to work flexible hours, including covering shifts as needed

  • Valid driver’s license and reliable transportation

Key Responsibilities

  • Conduct routine and as-needed site visits for home health patients and group home locations

  • Complete and oversee Plans of Care in compliance with physician orders and regulatory standards

  • Serve as the primary clinical liaison between staff, physicians, patients, and families

  • Provide clinical guidance and supervision to nursing staff

  • Perform internal audits to ensure compliance with home health and group home regulations

  • Monitor quality of care and implement improvements as needed

  • Coordinate care transitions and follow-ups with providers

  • Assist with training and onboarding of clinical staff

  • Build and maintain strong relationships with case managers, discharge planners, and referral sources

  • Support marketing and census growth efforts through professional outreach and relationship development

  • Represent the organization in the community and with healthcare partners

  • Transport medically complex patients to physician appointments when necessary

  • Fill in for nursing staff during call-offs or staffing shortages

Performance Expectations (KPIs)

  • Maintain and grow patient census through active relationship management and outreach

  • Establish and maintain referral relationships with case managers, hospitals, and community partners

  • Meet monthly/quarterly referral and admission targets

  • Ensure 100% compliance with OASIS and documentation standards

  • Maintain high patient satisfaction and quality scores

  • Complete audits with minimal deficiencies and implement corrective actions promptly

  • Ensure timely completion of Plans of Care and documentation

  • Maintain appropriate staffing coverage with minimal disruptions

Additional Responsibilities

  • Ensure documentation accuracy and timeliness

  • Maintain compliance with all state and federal regulations

  • Support operational needs across multiple locations

Compensation

  • Starting Salary: $100,000 annually

  • Compensation based on experience

  • Performance-based bonus tied to census growth, referral development, and quality metrics

    • Bonus eligibility based on meeting or exceeding monthly/quarterly census targets

    • Additional incentives may be awarded for strong audit outcomes and patient satisfaction scores

This position is intended for experienced home health nurses only. Candidates without direct home health and OASIS experience will not be considered.
