About the Role:

We are seeking a compassionate and skilled LPN to join our care team. As an LPN, you will provide direct care to our residents, including administering medications, implementing care plans, and monitoring resident well-being. You will work closely with our interdisciplinary team to ensure the highest quality of care and support for our residents.

Responsibilities:

Provide direct care to residents, including administering medications, performing vital signs, and monitoring resident condition
Implement care plans in collaboration with other healthcare professionals
Communicate effectively with residents, families, and healthcare team members
Participate in quality improvement initiatives to enhance resident care and experience
Maintain accurate and up-to-date records and documentation
