LPN/RN Nurse Case Manager Responsibilities:

Assess patients' needs and develop individualized care plans to promote optimal health and well-being
Coordinate with healthcare providers, families, and caregivers to ensure seamless transitions and continuity of care
Monitor patients' progress, make adjustments to care plans as needed, and communicate effectively with all stakeholders
Provide education and support to patients, families, and caregivers to promote independence and healthy habits
Collaborate with interdisciplinary teams to develop and implement effective care strategies and protocols
Document all patient interactions, assessments, and care plans in accordance with facility policies and regulations
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

LPN/RN Nurse Case Manager Requirements:

Current LPN/RN license in the state of New York
Prior experience in a healthcare setting, preferably in a case management role
Strong assessment, communication, and problem-solving skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks
Strong attention to detail and organizational skills
