Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee Hospitals throughout Morris County . Responsibilities: Complete an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated RN counsels the patient and family in meeting end of life needs Registered Nurse initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's Requirements: Current NJ RN License in good standing One year Hospice Experience REQUIRED One year of Hospital Setting Experience REQUIRED Willing to travel to the assigned areas Must have reliable transportation with liability insurance Strong communication and interpersonal skills. We are currently seeking experienced and compassionate supervisory nurses to join our team of dedicated professionals. Pay per visit per diem openings. RN Supervisor Responsibil ities: Collaborate with the Patient Coordinator and Clinical Manager to ensure accurate clinical documentation for reassessments and admissions. Initiate and revise the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Administration Records (TAR) as needed. Conduct monthly client visits both in-home and telephonic as required. Provide in-home supervision every 60 days or as necessary. Participate in formal and informal problem-solving processes to address EMR computer education needs and achieve established outcomes for users. Act as a liaison between Summit and the community, clients, facilities, etc. Evaluate the outcome of care and provide support to ensure that services meet the client’s needs. Inform Managers and other staff of changes in the client’s needs. RN Supervisor Requirements: Current RN license in the state. Willing to travel up to an hour in distance. Schedule visits with clients and families before due dates. 2+ years of supervisory experience. Previous experience in Home Care. Exceptional staff training and customer service skills.
