SUMMARY:
The Nurse Practitioner (NP) provides health care to patients and their families through the application of advanced knowledge and skills. In collaboration with the Medical Director, the NP provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient. The NP works collaboratively within the Interdisciplinary Group (IDG) and acts as resource person for staff and others on care related issues.
QUALIFICATIONS & ATTRIBUTES:
Master of Science in Nursing, minimum one year as NP, and five years as RN.
Preferred experience in long­ term care, oncology, or hospice.
Active RN license in state of practice and current certification as an NP required. Certificate as Hospice and Palliative Nurse preferred.
Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers.
Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.

MAJOR AREAS OF RESPONSIBILITY:
Performs patient visits and patient recertification activities such as the face to face encounter in coordination/ collaboration with the IDG and/or Medical Director.
Actively participates in IDG. Shares, presents and records activities with other members and directs patient's plan of care as needed. Ensures that documentation is compliant with organizational and governmental standards.
Works collaboratively with staff, referring physicians, attending physicians, medical directors, and the caregiving team concerning care of patients and potential referrals.
Assists in the review of patient clinical profiles including medication and care plan review to assess appropriateness and make adjustments as needed.
Provides cover for the physician when they are absent, to include answering patient calls, medication refill requests and other matters related to patient care.
Educates caregivers, co-workers and the community on all aspects of palliative care and services provided by agency.
Participates in developing current procedures regarding the provision of clinical care.
Attends and participates in internal team meetings as needed.
Prepares and maintains accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.
Prescribes and orders treatments and diagnostic tests according to licensure/credentialing body.
Assists in development of clinical practice guidelines I standards in support of quality palliative care as requested by supervisor.
Participates in Quality Assurance Performance Improvement (QAPI) activities.
Educates staff and patient/family on physical, emotional and psychosocial aspects of care, the disease process, symptom and pain management, signs and symptoms of death.
Demonstrates expertise in pain management and symptom control as related to terminally ill patients.
Accountable for proper utilization of hospice re­ sources.
Assists with occasional nursing visits, administrative on-call duties, referrals and admissions as needed.
Accountable for proper utilization of hospice benefits. Reports allegations of patient abuse, neglect, and/or misappropriation of patient property.
Follows Standard Precautions and infection control procedures when in a clinical environment.
Assures safety needs of the patients are addressed and met.
Assists with on-call duties, referrals and admissions as needed.
Demonstrates knowledge and competence in caring for hospice patients.
Actively promotes an open line of communication and a professional relationship with patients/families, facility staff, all members of the hospice team and administration.
Utilizes appropriate resources to resolve problems.
Acts as a representative for the company in promoting hospice education in the community.
Potentially travels to multiple locations during a work day.
Attendance is an essential job function.
Complies with all local/state/federal regulations and standards of their profession.
Complies with all company policies and procedures.

HEALTH QUALIFICATIONS:
Health Requirements: Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the job functions listed below. Bloodborne exposure risk is all.
