Overview

Works under the supervision of Registered Nurse in care of the patients and their environment (including those needing special precautions) and performs specified treatments. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served in assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and provide the care needed as described according to the departmental policy and procedure. Promotes and maintains safe work environment for staff as well as our customers, incorporating national patient safety initiatives. The LPN may work in a variety of settings and roles within Riverside Health System. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.

What you will do

  • Under the supervision of the RN, partners with patients and families to implement plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination of care. Facilitates safe patient transitions to the next care setting as part of plan of care.
  • Facilitates patient throughput to assure patients receive consistent, competent, and ethical care. Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence based practice and quality measures.
  • Provides nursing care using effective nursing skills (vitals, blood sugars, administering treatments and medications) in completing task accurately and efficiently. Follows protocols and physician orders.
  • Understands diverse patient care populations and modifies educational approach to ensure patient/family comprehension. Evaluates for patient comprehension and documents response.
  • Builds trusting collaborative relationships with peers and multi-disciplinary team to support patient care needs.
  • Completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
  • Prioritizes tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems/resources to obtain assistance.
