Overview

In addition to general nursing, is responsible for the delivery of safe, effective, and quality patient-family centered care in the OR and other areas of perioperative services for all patient populations.

Responsibilities

Assesses patients at intervals as stated in policies and procedures.
Functions in the scrub and or circulating role during operative or other invasive procedures in accordance with scope of practice, credentials, privileges, education, and experience
Ensures the operative procedure is being performed on the correct patient and site, side and level by: Reviewing the operative schedule; Reviewing the operative consent; Verifying procedure site and side with patient; Resolving identified discrepancy with surgeon and patient, prior to the patient being taken to the operating room; and participating in the pre-procedure verification, per-incision “Time Out” and sign out process.
Develops an individualized perioperative patient plan of care. Implements the plan of care by: protecting the patient from injury caused by chemical, electrical, laser, mechanical, and thermal sources and extraneous objects; performing accounting procedures to protect the patient from unintended retained surgical items; performing interventions necessary to ensure that the patient’s procedure is performed on the correct site, side, and level; managing the patient’s specimens in the appropriate manner; communicating the patient’s status to the appropriate parties throughout the continuum of care; administering medications safely and correctly; performing interventions to maintain the patient’s wound and tissue perfusion at or above baseline levels; performing interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems and fluid, electrolyte, and acid-base balances at or above baseline levels;
Providing education regarding the expected psychosocial response, nutritional management, medication management, pain management, wound management, and expected responses to the operative or invasive procedure;
Actively involving the patient or designated support person in decisions affecting his or her perioperative plan of care and the rehabilitation process.
Documents nursing care accurately, completely, and legibly.
Collaborates effectively with other disciplines as appropriate.
Participates in quality review and performance improvement projects.
Uses problem-solving and conflict resolution skills to foster effective work relationships with team members.

Qualifications

Education:

Internal & 1199 SEIU:

- Graduate of accredited nursing program(s).
- Bachelor's Degree in Nursing preferred.

Non 1199 SEIU Candidates:

- Bachelor’s Degree in Nursing.
- Master's Degree in Nursing, preferred.

Experience:

Minimum two (2) years' operating room experience.

Knowledge and Skills:

Able to read, write, see and hear English.

Licenses, Certifications:

Current NY State RN License required.

Pediatric Advanced Life Support (PALS) from the American Heart Association, required.
Certified Nurse, Operating Room (CNOR) certification, preferred.

Physical Requirements:

Physically demanding, high-stress environment requiring prolonged periods of standing, reaching, walking throughout the working day.
Position will be required to stoop, bend, lift and carry items weighing up to 35 pounds for routine performance of essential functions.
An individual in this position may be exposed to: Adverse indoor environmental conditions, Blood borne pathogens, Blood and body fluids, Physical hazards (e.g., needle pricks, etc.), communicable diseases, chemical hazards, radiation, and repetitive motions.

Reports and records same as appropriate; Keeps accurate documentation of patient's treatment, activity and condition, as well as patient's responses to medical and nursing interventions; Uses appropriate methods of documentation according to departmental policy; Acts rapidly and effectively during any emergency situation, managing self, patients and other employees; Provides a calm, quiet, restful atmosphere. Communicates effectively with the patient's family or caretakers; Participates in planning for discharge and coordinates referrals, as appropriate; Provides discharge instructions to patient and significant other. * Evaluates care provided for patient outcome Demonstrates ability to measure effectiveness of care provided and documents same; Performs variance analysis related to outcome data for performance improvement; Designs, implements, and evaluates systems to improve care in Unit; Keeps accurate documentation of patient's treatment, activity and condition; Uses appropriate methods of documentation according to departmental policy. * Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team Collaborates with the appropriate physician on patient's plan of care; Accurately interprets and implements treatment regimen as prescribed by the Physician; Assists the Physician during treatments and/or diagnostic procedures; Keeps the Patient Care Manager/designee and/or physician, abreast of changes in patient's condition and/or treatments, as appropriate; Uses clinical judgment in delegating assignments to the Patient Care Associate, Support Care Associate and Clerical Service Associate in providing patient care, and ensures that assignments are completed in a timely fashion * Performs grade I-IV Decubitus Care Performs preventive skin care measures Applies simple dressings, maintaining principles of aseptic technique; Applies warm & cold compresses, consistent with facility procedure; Performs irrigations, consistent with Facility procedure; Performs other procedures related to skin care, as necessary. * Administers medications correctly and safely Correctly identifies medication: action; dosage; side effects and implications for the patient; Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. * Participates in patient and family education Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-hospital care; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Performs related duties, as required. ADA Essential Functions Job Qualification Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after January 1, 2013 and obtain a BSN Degree within five (5) years of employment date. Current license to practice as a Registered Professional Nurse in New York State. BLS required. Documented OR experience required. On Call required.
