DUTIES:

Provides advice & support to the Residents and their families.
Monitoring Residents and administering medication and treatments.
Documents Resident care services by charting in Resident & dept. records.
Protects Residents & staff by adhering to infection-control policies & protocols.
Resolves Resident problems &needs by utilizing multidisciplinary team strategies.
Assures quality of care by adhering to Facility philosophies & standards of care.
Supervising LPNs and Certified Nursing Assistants (CNA)
Maintains Resident confidence by keeping information confidential.

REQUIREMENTS:

Should work well in a team-oriented environment.
Must hold a valid RN License.
Strong computer skills.
Excellent communication skills.
Long Term Care experience a plus.
Should be friendly and a team worker.

Accurately records patient information in the electronic medical records system and patient medical record. Demonstrate a current working knowledge of equipment, instrumentation and medications. Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals. Participate in preparation and facilitation of regulatory agency site visits to include DOH In accordance with agency protocol and DOH standards, maintains adequate infection control standards. Reviews and reports all medical, accident/incidents related to patient care and provides input into actions necessary to resolve problems. Participate in weekly and on an “as needed basis” case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns. Ensure that personnel adhere to established policies and procedures. Document changes in resident's conditions. Participate in care planning meetings. Mentoring and supervision of LPN's and CNA's. Requirements: Current NYS Registered Nurse license. 3 Years RN experience preferred. Current BLS Certificate preferred.
