We are seeking a highly skilled and compassionate RN/LPN Case Manager to join our team in Fort Myers, Florida. As a member of our rehabilitation and skilled nursing services team, you will play a vital role in ensuring our patients receive the highest quality care and support to achieve their goals.

RN/LPN Case Manager Job Summary:
As an RN/LPN Case Manager, you will be responsible for coordinating patient care activities, developing and implementing individualized treatment plans, and providing emotional support to patients and families. Your expertise in rehabilitation and skilled nursing services will enable you to identify and address patient needs, facilitating timely transitions to the next level of care. Your passion for providing exceptional patient-centered care will shine through in your interactions with patients, families, and healthcare team members.

RN/LPN Case Manager Responsibilities:

  • Coordinate patient care activities, including assessment, planning, implementation, and evaluation

  • Develop and implement individualized treatment plans, incorporating clinical and therapeutic interventions

  • Provide emotional support and education to patients and families, promoting independence and self-advocacy

  • Identify and address patient needs, facilitating timely transitions to the next level of care

  • Collaborate with interdisciplinary healthcare teams to ensure seamless communication and coordination

  • Document patient care activities, progress, and outcomes in accordance with regulatory requirements and facility policies

RN/LPN Case Manager Ideal Candidate:

  • Current Florida RN or LPN licensure

  • Prior experience in rehabilitation and skilled nursing services

  • Experience with insurances required.

  • Experience with MDS preferred.

  • Strong clinical and interpersonal skills

  • Ability to work effectively in a fast-paced, dynamic environment

  • Excellent communication and documentation skills
