Licensed Practical Nurse (LPN) – Home Health
We are expanding our clinical team and seeking dependable, skilled Licensed Practical Nurses (LPNs) to provide high-quality care to medically complex clients in the home setting. This role offers the opportunity to deliver meaningful, one-on-one patient care while working under the supervision of a Registered Nurse and physician.

Service Areas:
Tampa, Brandon, Valrico, Lakeland, Mulberry, Dover, Lithia, Riverview, Ruskin, Sun City Center, Seffner, and more.
Key Responsibilities
Provide skilled nursing care during home visits within assigned territories
Administer medications, treatments, wound care, and other ordered procedures
Deliver individualized care while evaluating patient response to treatment
Follow physician orders and the established plan of care under RN supervision
Monitor patient conditions and promptly report changes to the clinical team
Complete accurate, timely documentation in accordance with agency standards
Perform patient assessments as required
Assist with activities of daily living when appropriate
Educate patients and families on care needs, treatment plans, and disease management
Qualifications & Skills
Active, unrestricted Florida LPN license
Graduate of an accredited nursing program
Minimum 1 year of nursing experience preferred
Prior home health experience strongly preferred
Experience with:
Pediatrics, adults, and geriatrics
Feeding tubes (G/J tubes)
Developmental and neurological disorders
Endocrine conditions and injections
Tracheostomy care
Strong clinical judgment and ability to work independently
Effective time management and organizational skills
Proficiency with EMR systems (experience with Kantime preferred, but not required)
Solid documentation and communication skills
Understanding of infection control, safety standards, and patient confidentiality
Additional Requirements
Valid driver’s license and reliable transportation with active insurance
Current BLS/CPR certification
Recent statement of health including TB screening
Ability to travel to assigned cases
