Licensed Practical Nurse Boulder Park Terrace

The Licensed Practical Nurse (LPN) works under the direction of the Team Leader. He/she gathers assessment data, monitors, follows plan of care and promotes optimal level of health. The LPN provides medication administration, treatments, supervision of CENAs performing ADLs. Collaborates with the Care Coordinators and other team members to ensure communication and timely updates on residents change in conditions.

KEY RESPONSIBILITIES INCLUDE:

  • Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader. Reports findings to the Team Leader.
  • Provides competent care consistent with the nursing process and written plan of care, physician’s orders and in accordance with skilled nursing facility policies and procedures.
  • Participates in admission and discharge planning, in collaboration with health care team, at the time of admission and continues through discharge.
  • Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader.
  • Documents care consistent with guidelines.
  • Performs other duties consistent with purpose of job as directed.
  • Graduate of an accredited school of nursing.
  • License to practice practical nursing in the State of Michigan.
