Job Summary 

The LPN is responsible for delivering nursing care within their scope of practice, ensuring resident safety, and supporting the development and implementation of individualized care plans. 

 

Key Responsibilities 

  • Provide direct nursing care in accordance with facility standards and policies 

  • Assist in developing and updating care plans under RN supervision 

  • Monitor and record vital signs and report changes in resident condition 

  • Administer medications and treatments as prescribed 

  • Maintain accurate documentation of care and medication records 

  • Supervise and direct Nursing Assistants as needed 

  • Participate in daily rounds and communicate with physicians and care teams 

  • Perform treatments such as dressing changes, specimen collection, and other procedures within scope 

  • Ensure resident comfort, safety, and dignity at all times 

  • Follow infection control protocols and safety guidelines 

  • Maintain a clean, safe, and organized work environment 

  • Respond promptly to resident needs and call lights 

  • Attend required trainings and staff meetings 

 

Qualifications 

  • Graduate of an accredited School of Nursing 

  • Current Licensed Practical Nurse (LPN) license in the state 

  • Current CPR/BLS certification 

  • Strong communication, organizational, and clinical skills 

  • Ability to work effectively in a team environment 

  • Compassionate and resident-focused approach to care
