RN MDS Coordinator

We are seeking an experienced Registered Nurse (RN) MDS Coordinator to lead the MDS assessment process and help ensure quality outcomes, regulatory compliance, and accurate reimbursement. This is an excellent opportunity for an organized RN with long-term care experience who enjoys working with an interdisciplinary team.
What You'll Do
Coordinate and oversee the complete MDS/RAI process, including MDS assessments, CAAs, and individualized care plans.
Ensure assessments are completed accurately and submitted on time in accordance with CMS and state regulations.
Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members to develop comprehensive resident care plans.
Maintain the MDS assessment schedule for all payer sources, including Medicare and Medicaid.
Conduct resident assessments through chart reviews, interviews, and direct observation.
Monitor documentation to support accurate coding, reimbursement, and regulatory compliance.
Participate in PPS meetings, Medicare reviews, and quality improvement initiatives.
Assist with chart audits and provide education and guidance to clinical staff regarding documentation and MDS best practices.
Stay current on CMS regulations, PDPM requirements, and industry standards.
What We're Looking For
Active RN license in North Carolina.
At least 3 years of nursing experience, preferably in a skilled nursing or long-term care setting.
Previous MDS Coordinator experience strongly preferred.
Knowledge of CMS regulations, PDPM, Medicare reimbursement, care planning, and MDS coding.
Current CPR certification.
Excellent organizational, communication, and leadership skills.
Ability to manage multiple deadlines while maintaining accuracy and attention to detail.
