In addition to program development, the Heart Failure Nurse Navigator serves as a clinical liaison and advocate for patients with heart failure, ensuring seamless coordination of care across the continuum, from diagnosis through treatment and follow-up. This role involves patient education, care coordination, and support for both patients and their families, facilitating communication between multidisciplinary healthcare teams and helping patients navigate complex medical systems.

Education: Bachelor's Degree

Experience: Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.
