The Heart Failure Nurse Navigator will assist with planning, developing, implementing, and managing the Heart Failure Program under the direction of the Heart Failure Program Coordinator, Heart and Vascular Executive Director, and Cardiology Medical Director. In addition to program development, the Heart Failure Nurse Navigator serves as a clinical liaison and advocate for patients with heart failure, ensuring seamless coordination of care across the continuum, from diagnosis through treatment and follow-up. This role involves patient education, care coordination, and support for both patients and their families, facilitating communication between multidisciplinary healthcare teams and helping patients navigate complex medical systems.
Required Knowledge & Skills
Education: Bachelor's Degree
Experience: Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.
Licenses and Certifications
*GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE
*ACLS CERTIFICATION
*HEALTHCARE PROVIDER (CPR)
Qualification
*Bachelor's degree in nursing required. Masters preferred.
*Certification in Cardiac Nursing (e.g., CCRN, PCCN, CVRN) preferred.
*Meets and maintains the qualifications and certifications outlined.
*Excellent interpersonal and communication skills.
*Strong organizational and problem-solving abilities.
*Ability to work independently and collaboratively in a multidisciplinary setting.
