The Home Health RN provides skilled, compassionate care in participants’ homes under CMS guidelines, ensuring participants convenient access to essential health services where they feel most comfortable. Working closely with the interdisciplinary team (IDT), this role coordinates care plans to empower participants to live healthier, more independent lives.

This role is different because Home Health RNs at WelbeHealth:

On the day-to-day, you will:

  • Conduct home health nursing assessments to determine the nursing equipment needs in the home, including participant preferences and goals
  • Perform in-home urgent care level nursing assessments and interventions, including point-of-care testing (e.g., UTI dipstick analysis, COVID/respiratory swabs), vital sign monitoring, and rapid clinical evaluation to determine patient needs and escalation requirements.
  • Maintain on-call readiness and respond promptly to time sensitive visit requests, demonstrating the ability to mobilize quickly to patients’ homes and provide timely, high-quality care in dynamic situations.
  • Deliver and document home health nursing interventions as agreed upon in participant care plans, including but not limited to maintaining a healthy and safe environment, promptly responding to physician orders, and accurately administering medications and performing ordered tests and treatments
  • Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions
  • Track and monitor home health scheduling in collaboration with the Home Health Manager or scheduling point of contact
  • Collaborate with the Physician and other IDT members to assist in smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.)

Job requirements include:

  • Graduate of an accredited school of nursing
  • Unencumbered RN License required
  • Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred
  • Reliable means of transportation
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Position Overview We are seeking a compassionate and experienced Director of Nursing to lead our hospice clinical operations. This key leadership role is ideal for a Registered Nurse with strong hospice or palliative care experience and a heart for end-of-life care. The DPCS ensures exceptional care quality, regulatory compliance, and team coordination while supporting patients, families, and staff in a meaningful way. Key Responsibilities Clinical Leadership: Direct the delivery of hospice services in compliance with state, federal, and internal standards. Team Management: Recruit, train, supervise, and mentor clinical staff including RNs, LVNs, social workers, and home health aides. Care Coordination: Guide the interdisciplinary team in developing and executing personalized care plans. Regulatory Compliance: Ensure adherence to Medicare/Medicaid requirements and accreditation standards; manage audits and documentation. Quality Improvement: Drive initiatives to enhance patient outcomes and care quality. Budget Oversight: Manage clinical budgets and monitor resource utilization effectively. Family Support: Serve as a compassionate liaison for patients’ families, offering clear communication and emotional support. Cross-Department Collaboration: Partner with social services, spiritual care, and admin teams to provide holistic care. After-Hours Protocol: Develop and maintain readiness protocols for emergencies and after-hours needs. Qualifications Education: BSN required; MSN or advanced degree preferred. Licensure: Active Registered Nurse (RN) license in the state of California (required) . Experience: Minimum of 2 years in hospice or palliative care leadership.
