This APP will work with the Endocrinologist physician at the practice. The practice is open Monday to Friday from 8:00 am to 4:00 pm. Key Responsibilities: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients. Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries. Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management, and educate patients on self-management strategies. Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans. Collaborate with physicians, advanced practice providers, and nurses to coordinate patient care across multiple disciplines and healthcare settings.
