Overview:


 

1.0 FTE, 20 hours per week

Mondays and Fridays at Heinzerling Care Facility (1800 Heinzerling Drive, Columbus, OH 43223) to provide care to residents who have complex medical conditions. Will assist with calls related to concerns for these patients.

Family APRN preferred.

At least 3 years' experience as an APRN strongly recommended.

Primary location will be at Heinzerling Care Facility

Essential Functions:

  • Assesses patients and/or populations; develops, implements and evaluates plans of care; clearly documents and communicates information both in written and verbal forms.
  • Performs diagnostic and therapeutic procedures according to established guidelines and standards as necessary for the care of the patient.
  • Utilizing evidence based practice, serves as a clinical expert for policies, procedures, documentation, and skills.
  • Assists in the education, training and development of new and existing staff members.
  • Engages in the “community of the work unit” by staying aware of changes, participating in data collection or auditing, engages in Quality Improvement.
  • Leverages technology and informatics to optimize healthcare outcomes.
  • Commits to continued professional development of self and others.
  • Contributes to the division and/or specialty goals and the Hospital Strategic Plan.


Education Requirement:

Master of Science degree or equivalent provision.


Licensure Requirement:

Licensed as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the state of Ohio.


Certifications:

Maintains national certification as APRN in the appropriate population foci for the position, or PA.

Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP/PA-C) for Westerville Internal Medicine, 625 Africa Rd., Suite 200, Westerville, Ohio 43082. The APP, in collaboration with the Physicians, coordinates the health care of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress. About the practice: This COPC practice has established a reputation as a primary care group that serves the community. They provide the best primary care for all ages with a focus on health education, disease prevention, and management to keep their patients well. It is a large outpatient practice consisting of 5 Internal medicine physicians and an Endocrinologist. There is an Office Manager, an APP, several medical assistants, patient service reps, an RN, an LPN, a Clinical Lead, Clinical Assts, and a phlebotomist to support the providers and patients. This APP will work with the Endocrinologist physician at the practice. The practice is open Monday to Friday from 8:00 am to 4:00 pm. Key Responsibilities: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients. Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries. Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management, and educate patients on self-management strategies.
