Our patient census is expanding. Our team is growing. We are hiring a Clinical Team Manager (RN) to help lead and support our home health clinicians as we continue serving more patients in the community.

What you will do

• Oversee daily patient care services and clinical staff
• Assign clinicians and coordinate patient visits and referrals
• Ensure patient needs are assessed and care plans are developed and updated
• Coordinate care with physicians, vendors, and care teams
• Review and approve plans of care and physician orders
• Conduct OASIS reviews and ensure regulatory compliance
• Coach and mentor field clinicians
• Participate in interviewing and hiring clinical staff
• Monitor branch clinical performance and outcomes
• Support field visits when needed

What you need

• Associate degree in Nursing or higher
• Active RN license in Indiana
• 2 years home health or hospice RN experience
• 1 year leadership experience preferred
• Knowledge of Homecare Homebase and Microsoft Office preferred
• Reliable transportation, valid driver’s license, and current auto insurance
• Ability to lift, push, pull, or support up to 50 pounds

You will report to the Branch Director and play a key role in supporting our expanding clinical team and delivering high quality care to patients across the Valparaiso community.
