Administer medications and treatments as ordered
Monitor residents and report changes in condition
Maintain accurate and timely documentation
Assist with admissions, discharges and transfers
Participate in individualized care planning
Collaborate with residents, families and interdisciplinary team members
Follow infection control procedures and maintain a safe care environment
Provide compassionate, person-centered care

Qualifications:

Active Licensed Practical Nurse (LPN) license in good standing
Strong communication and interpersonal skills
Compassionate approach to resident care
Ability to work collaboratively with residents, families and the healthcare team
Ability to work effectively in a skilled nursing environment
