  • Core Job Functions
    • Documents all patient related clinical information and assesses patient for special needs and plans appropriate intervention. Comprehensive review of the patient's medical history is performed and documented.
    • Assessment and reassessment are completed and changes in status are communicated to appropriate care provider. Plan of care is formulated and modified based on evaluation. Collaborates with disciplinary health care team to improve patient outcomes.
    • Contributes to formulation of nursing careplan for patients in consultation with the RN and in collaboration with patient, families, healthcare members using established nursing diagnoses and nursing protocols. Prioritize nursing care needs of clients. Assist in the review and revision of nursing care plans with the RN to meet the changing needs of the patient.
    • Assess data related to basic, physical, developmental, spiritual, cultural, functional, and psychosocial needs of the patient. Collects data within established protocols and guidelines from various sources. Documents data collection, assessment, and communicates findings to appropriate healthcare members.
    • Care plan development and implementation with health care team members collaborating to develop long and short term patient goals. Communicates with Physician using Situation, Background, Assessment and Recommendation format. Contributes to the interdisciplinary team to manage Wound Care patients.
    • Patient documentation is accurate and timely and it reflects rationale for any deviation from standards of care. Practice is congruent with evidence based wound care guidelines.
    • Observing Universal Protocol at all times provides clinical assistance to patients and notifies appropriate medical personnel of potential critical patient conditions. Ensures coordination and continuity of care with community and medical resources as indicated.

Position Summary In collaboration with the Manager of Clinical Operations for Patient Placement, Nursing Unit Clinical Managers and the Directors of Patient Care Services, the RC is responsible for the proactive planning, implementation and coordination of daily patient placement and throughput activities to maximize available bed space and access.
